Our two-page 2012 Liability / Medical Form must be filled out and signed before skating at Skatechurch. A parental signature is required on both sides of the form. A notarized signature is required on page 2. A parental signature in person in front of an authorized Skatechurch staff person is acceptable instead of a notarized signature.
