Our two-page 2008 liability / medical form must be filled out and signed
before skating at Skatechurch. Note: A parental signature is required
on both sides of the form. A notarized signature is required on page 2. Parental
signatures in person in front of an authorized Skatechurch staff person are acceptable
instead of a notarized signature. Please contact
